For your appointment, please remember to wear comfortable attire, such as activewear or shorts, that allows visibility of the area you'd like us to address in your exam. Should you forget, we're happy to provide you with a gown.
In this section of our website, you can save yourself time on your appointment day by completing your New Patient Forms prior to arrival. If you prefer to fill out these forms by hand, we can provide you the forms in office prior to the start of your appointment. In order to allow providers to maintain their scheduled appointments and to respect all of our patients' time, please be sure to arrive 15-20 minutes prior to your scheduled appointment time to fill out these forms.
